I.Consent for the Collection and Use of Personal Information

Korea International School / Korea International High School collects, uses, and processes personal information in accordance with the Personal Information Protection Act. The collected personal information will not be used for any purpose other than those stated and will be managed in accordance with applicable laws and regulations.

You may choose not to consent to the transfer of personal information overseas. However, without your consent, you may not be able to access certain educational services provided by international platforms such as the school email system (Google) or the academic management system (PowerSchool). PowerSchool is a cloud-based academic management system based overseas. All data is encrypted during transmission and storage. The system complies with international data protection standards such as FERPA (U.S. Family Educational Rights and Privacy Act) and GDPR (General Data Protection Regulation in the EU). Default security protocols are applied even if the school does not implement additional encryption settings.

■ Consent for Use of School Systems

To use school email (Google email system), PowerSchool, and other school-managed systems, I have read and agree to the following:
1. The provided account must be used solely for academic and school-related purposes.
2. I will comply with the school’s security and operational policies. Violation of these policies may result in restrictions on account usage.
3. If I do not consent to the use of school systems, I may not be granted access to those systems.

■ Consent to School Policies

I have read and agree to the following school policies.
1. I understand that if I violate school rules during school-related activities or events (such as field trips, after-school programs, outdoor activities, sports, etc.), teachers or coaches may take disciplinary actions including removal from the activity and/or restrictions from future participation.
2. I agree that photos and videos (for which I have given consent to collect personal information) may be used on the school website, social media platforms, etc., for the purpose of school life communication and promotion.
3. I understand and agree that the medical information provided in the Medical History Form (to which I have consented as sensitive information) may be shared with student management staff (teachers, nurses, school meal providers) for the purposes of student care and academic support.
4. I acknowledge that attendance policies are subject to change, and all absences will be closely monitored by teachers and the school. Repeated unexcused absences may result in further action.

■ Consent for Emergency Contact Update and Submission of Medical History Form

I agree to update and submit the Emergency Contact Information, Certificate of Family Relations, and Medical Information (Medical History Form) annually for the health and safety of the student.
1. Changes in the student’s health status must be accurately recorded.
2. The parent/legal guardian must provide a valid and up-to-date phone number to enable prompt contact in case of an emergency, and must inform the school immediately of any changes.
3. A secondary emergency contact designated by the parent/legal guardian must also be provided in case the primary contact cannot be reached.

4. In accordance with Article 909 of the Korean Civil Act (Joint Exercise of Parental Authority), the emergency contact information of one parent/legal guardian cannot be removed based solely on a request from the other parent.

■ Consent for Student Supervision and Monitoring on School Premises

I agree to the school policy that requires students to remain within the supervision of school staff to ensure student safety on campus.
1. Students must stay within designated areas of the school, even outside of class hours.
2. Moving to unauthorized locations without staff supervision or permission may result in disciplinary action.
3. This policy is enforced with the student’s safety as the school’s highest priority.

■ Consent for Locker and Personal Belongings Inspection

I have read and agree to the following regulations regarding the inspection and management of student lockers and personal belongings.
1. The school has the authority to inspect and manage student lockers when there is an urgent need or other special circumstances in order to ensure student safety.
2. If there is reasonable suspicion that a student is in possession of items that could seriously disrupt the learning environment or the safety of the dormitory—such as alcohol, tobacco, drugs, explicit materials, or electronic devices that could be used for misconduct—the school reserves the right to inspect lockers, dormitory spaces, and personal belongings regardless of the student’s consent. A female staff member will be present during inspections involving female students.
3. If any illegal or prohibited items are found, appropriate action will be taken in accordance with school regulations.

■ Consent to the Electronic Device and Internet Use Policy

I have read and agree to the following electronic device and internet use policy.

1. Restrictions on Electronic Device Use
• The use of personal electronic devices for non-academic purposes is restricted during class time.
• During exam periods, the use of electronic devices is strictly prohibited in testing areas to prevent cheating.
• Electronic device use may be restricted in designated areas such as the library, auditorium, and other school-specified zones, and students must follow posted guidelines.

2. Monitoring and Inspection of Electronic Device Use
• If a student’s use of an electronic device violates school policies or negatively impacts learning or school life, the school may request an inspection within a reasonable and minimal scope.
• In principle, inspections are conducted with the consent of the student and parent/guardian and will be explained to the student in advance.
• Staff may not search a student’s electronic device without the student’s consent. However, in cases requiring legal action (e.g., criminal suspicion, emergency situations), appropriate authorities may be involved.

3. Regulations on the Use of AI Programs and Social Media
• AI programs (e.g., ChatGPT) may only be used within the scope permitted by the teacher for classwork and assignments.
• Students must not violate others’ privacy or post hate speech on social media. Any such violation may result in disciplinary action according to school policies.
• The school will not inspect students’ personal social media accounts arbitrarily. However, if a specific post significantly affects the school community, the school may request its removal and take appropriate disciplinary action.

■ Consent for Medical Treatment and Administration of Medication

I have read and agree to the following medical-related policies for the purpose of student health management and emergency response.
1. I consent to my child receiving medical treatment in the event of illness or injury while at school. In emergency situations, I allow school staff to assist the student until professional responders (such as emergency medical personnel/119) arrive.
2. School staff may take the student to a medical facility or purchase/administer prescribed medication as necessary.
3. I understand that the school is not responsible for any incidents resulting from medication prescribed by a physician or treatment administered by professionals (such as doctors or paramedics), and I acknowledge that all related costs (such as hospital or transportation fees) will be billed to the parent/guardian.
4. I understand that the school is not responsible for any incidents caused by medications provided by the parent/legal guardian.
5. I acknowledge that all medications provided to the school must be submitted in their original containers with intact labels.
6. I consent to the school nurse or designated staff (e.g., staff on duty) administering the medication(s) listed below at school.
7. I understand and agree that medical information collected as sensitive personal data may be shared with designated staff (e.g., homeroom teacher, subject teachers, counselors, principal, school nurse) for the purposes of student care (counseling, academic support, etc.) and health management.
8. I agree to follow the school’s infectious disease response protocols and submit all documents requested by the school nursing team. I also understand that all associated costs (e.g., medical fees, transportation) for hospital visits will be billed accordingly.

■ Consent for the Use of Photographs and Videos

I have read and agree to the following regarding the use of photographs and videos.
1. I understand that photographs and videos of the student may be used by the school for promotional or educational purposes, even after the student graduates or transfers.
2. When using photos or videos that include the student, the school will use the student’s image as originally captured, without alterations.
3. The student’s photos and videos will not be used for any commercial purposes beyond school promotion or education.

■ Consent Regarding Off-Campus Activity Conduct and Response

I have read and agree to the following terms regarding off-campus school activities.
1. I understand that if a student fails to comply with school rules during an off-campus activity, the student may be sent home at the parent/legal guardian’s expense, and additional disciplinary measures may follow.
2. If a student is required to return home from an off-campus location, the parent/legal guardian must personally pick up the student at the activity site.
3. In cases where a student cannot be immediately handed over to the parent/legal guardian, I understand that the student may be temporarily separated and supervised in a designated space.
4. I acknowledge that all instructions and requests from teachers (chaperones) are for the student’s safety and well-being, and students must follow all school rules. Failure to comply may result in being sent home or subject to further disciplinary action.
5. I agree that the parent/legal guardian will be responsible for all expenses resulting from any damages or injuries caused by the student’s failure to follow the teacher’s instructions, including property damage and any physical harm or medical expenses.
6. I agree not to engage in smoking (including e-cigarettes), drinking, violence, bullying, or inappropriate sexual behavior during school-sponsored off-campus trips.

■ Consent for Academic Support and Counseling

I have read and agree to the following terms regarding academic support and counseling.
1. I consent to the student receiving academic, social/emotional, college/career counseling, and other support services provided by the school.
2. I understand that information shared during counseling may be disclosed under the following circumstances:
• If there is a serious risk of harm to oneself or others
• If there is evidence or disclosure of abuse (physical, emotional, or sexual) or neglect
• If there is a threat to school safety
• If there is an ongoing criminal or delinquency-related legal proceeding

※ The school counselor or professional will inform the student of these limitations to confidentiality and will notify the student when information needs to be shared with others.

■ Consent to the KISJ ES/MS/HS School Handbook 2025-26

I have read and agree to the following contents of the KISJ ES/MS/HS School Handbook 2025-26.
1. I confirm that I have read and understood the information provided in the KISJ ES/MS/HS School Handbook 2025-26 .
2. I agree to pay all fees set by the school in full by the specified due dates, in accordance with the approved payment plan. I understand that failure to make payment may result in restrictions such as being barred from attending classes, withholding of report cards, or denial of re-enrollment.
3. I agree to submit supporting documents for absences.
4. The parent/legal guardian agrees to cooperate with teachers and administrative staff to support the student’s academic progress.
5. I agree to follow the proper communication procedures established by the school when raising concerns or issues (i.e., first contacting the teacher or staff member most directly involved, then escalating to the principal or dormitory head if needed).
6. I understand that shuttle bus, cafeteria, and laundry services are provided by third-party vendors and that the school acts only as an intermediary. I agree to comply with the policies and financial obligations set by these vendors.
7. I understand my responsibility to contribute to a positive school environment and to refrain from any behavior that causes intolerance or discord among community members, or from making unfounded statements that may harm the reputation of the school or its staff.
8. I acknowledge that failure to follow KIS Jeju policies and guidelines may result in disciplinary actions, including expulsion.

By providing my name and signature, I confirm that I have received, read, and understood all policies outlined in the KISJ ES/MS/HS School Handbook 2025-26 and agree to comply with them.

■ Consent to Dormitory Regulations

I have read and agree to the following dormitory-related policies outlined in the KISJ ES/MS/HS School Handbook 2025-26.
1. I confirm that I have read and understood the dormitory regulations included in the KISJ ES/MS/HS School Handbook 2025-26.
2. Upon entering the dormitory, both the student and the parent/legal guardian must comply with all dormitory rules.
3. Dormitory regulations apply equally to students aged 18 and above, and any violations are subject to disciplinary action by the school.
4. Failure to follow shared living rules in the dormitory may result in warnings or expulsion from the dormitory.
5. I understand that serious misconduct such as sexual or physical violence in the dormitory will lead to immediate expulsion from the dormitory.
6. I will not smoke (including electronic cigarettes) or possess smoking-related items anywhere on campus.
7. I will not consume or possess alcoholic beverages anywhere on campus.
8. Consent to safety and emergency procedures in the dormitory – In the event of an emergency where there is a delay in contacting the guardian, dormitory staff and supervisors may take appropriate action.
9. Consent to dormitory infection control and disease prevention policies – In case of a diagnosis of an infectious disease, I agree to isolation procedures and the use of a single room as required.
10. Consent to personal belongings inspection policy – I understand that dormitory supervisors may inspect students' belongings when necessary and, depending on the situation, may do so without prior notice.
11. Restrictions on off-campus outings and overnight stays may be implemented to maintain order and ensure student safety.
12. I understand that records and confirmations of off-campus outings and overnight stays may be kept for student safety and dormitory management.
13. I understand that dormitory behavior guidance and counseling records may be maintained to address and resolve issues that arise in the dormitory.
