2024-25 Agreement to Collect and Use Private Information

Korea International School, Jeju Campus is required to collect the following personal information from student/parent (legal guardian). According to the "Personal Information Protection Law", KISJ needs your consent on collecting, using and providing the following. If you agree to the collection and purpose of the items, please check the boxes below and sign the agreement

 

 

1.Personal Information Collection and Usage

1) Collected Items

  1. Student information: name, resident registration number/alien registration number, gender, address, school, grade, passport information, school record, letter of recommendation, sibling’s information (name), Parent (guardian) information: name, date of birth, phone number, email address, nationality, passport information (foreign passport holders only), occupation, position, name of company, address
  2. Photo, Video
  3. Parent’s bank account number
  4. Assessment of Student Health, Medical Form

2) Purpose

  1. Manage Academic Support & Admissions Application
  2. For the use of advertising and publications such as yearbook, brochures, etc.
  3. For refund purposes
  4. To prepare and provide proper primary treatment in case of emergency and check for allergies

3) Period of Retention

  1. Collected Item 1: Semi-permanent (Article 25, Elementary and Secondary Education Act), Denied/not enrolled (1 year)
  2. Collected Item 2: After student transfers out/ Alumni (1 year)
  3. Collected Item 3: Until student transfers out/Graduation
  4. Collected Item 4: After student transfers out/Alumni (5 years)

2.Providing personal information to 3rd party

1) Organization and Purpose

  1. Korea International School: carry out the task of screening, sharing database system to facilitate the management of academic support.
  2. Korea International High School: carry out the task of screening, sharing database system to facilitate the management of academic support.
  3. PowerSchool: Academic records system
  4. MAIA Learning: University admission application system
  5. Google: Management of Student Information

2) Items of personal information collection: same as Category 1

3) Period of retention and use of collected personal information: same as Category 1

 

3.Providing personal information for overseas transferring students

Korea International School Jeju uses the following programs for registrar services:

1) For the use of personal information

  1. PowerSchool: same as Category 1 (Resident registration number/alien registration number, excluding passport number)
  2. MAIA Learning: name, grade, school record
  3. Google: Registrar service and administrative process

2) Countries where personal information is transferred, date and procedure

  1. PowerSchool: USA, newly enrolled, network service
  2. Google: USA or European countries (abide Google’s internal regulations, newly enrolled, network service

3) Companies where personal information is transferred to

  1. PowerSchool: International Support / +1 916-288-1881
  2. MAIA Learning: MaiaLearning Inc. / +1 855-268-2771
  3. Google: Google cloud support team (Data Protection Team for Cloud Support / +1 646-257-4500

4) Period of retention of Personal Information

  1. PowerSchool: registrar service program, semi-permanent
  2. MAIA Learning: college application system
  3. Google: registrar service and administrative process, same as Category 1’s period of retention and purpose (personal information collection (Deleted after 25 days of disposal)

 

4.Agreement to Process Personal Information to outsourcing organization

School consigns personal information for the successful working process and related information is held until the consignment is completed. Companies may change according to the contracted term. Any changes that occur will be posted on the website.

5.Right to withhold consent of retention and usage of personal information

You have the right to withhold consent. In case of refusal of consent to collection of the required information, It may create a blockage to student application and admittance procedures.

 

Ⅰ. KISJ Parent/Guardian Permission for Learning Support and Counseling

KISJ provides learning, counseling, and other school-sponsored support to students based on the Academic, Career, Personal/Social, and Global Perspective Domains.

 I give permission for my child to receive academic, social/emotional, college/career counseling, and/or any other support at Korea International School, Jeju Campus.

 All services at KISJ are governed by the American School Counseling Association (ASCA) and its International Model for School Counselors, as well as the National Association of Special Education Teachers (NASET) Code of Ethics. Please see the KISJ School Counseling Program Information booklet.

 Confidentiality*

I understand that the school learning specialist will keep information confidential, with the following exceptions.

  1. Presenting a serious danger to self or another person;
  2. Evidence or disclosure of abuse (physical, emotional, sexual) or neglect;
  3. Threats to school security;
  4. Pending criminal or delinquency proceedings.

 *The school counselor or any other specialist will make the child aware of these limits to confidentiality and will inform the child when sharing information with others.

 

Ⅱ. KISJ Student Off-campus Travel Value Contract

This contract establishes expectations for KISJ students on school-organized travel experiences, which are designed to offer students learning opportunities in a non-classroom setting. All KISJ School and Dorm Life Guide school rules are in effect during trips. Each participant is responsible for making the trip a positive, safe, and enjoyable experience for the entire group by demonstrating high standards of behavior, accepting responsibility for their actions, and exhibiting courtesy and consideration.

 I pledge to honor KISJ during any off-campus travel. I pledge to be responsible by:

  1. Following all directions/rules/regulations from group leaders before and during the trip
  2. Behaving, speaking, and dressing appropriately and respectfully at all times
  3. Acting with maturity and thinking carefully before making decisions
  4. Speaking our connecting language (English) at all times
  5. Arriving to all meeting and check-in times and locations early
  6. Showing a positive attitude and trying my best in every activity
  7. Remaining in a group of 3 or more at all times, and never going anywhere alone for my safety
  8. Sleeping in my assigned accommodation for the entirety of each night
  9. Refraining from smoking, drinking alcohol or partaking in any type of illegal substances, stealing, mixing company (boys and girls in the same room), leaving the premises without permission or supervision or involving myself in any type of behavior detrimental to the health, well-being, safety, or reputation of myself or the group, including the group leaders and/or KISJ

 I pledge to be respectful by:  

  1. Conducting myself with the highest level of etiquette at all times
  2. Recognizing aspects of the host culture, and acculturating by learning gestures of respect and attempting to speak basic phrases in the country’s language
  3. Being kind, considerate and mindful of local culture
  4. Remaining calm and quiet on buses, trains, planes, boats
  5. Following the instructions of my teachers, tour guides and other adults who are a part of the international learning experience, cooperating with their instructions
  6. Leaving all places of accommodation in the same condition in which they were found, acting in a mature/courteous manner, keeping voice levels at a minimum, not misusing hotel phones

 I pledge to be a leader by:

  1. Being a role model to my friends and helping them to uphold their pledge
  2. Paying attention to people, places, and the culture around me
  3. Being responsible for myself and volunteering

Both the student and the parent/guardian realize failure to follow the above stated expectations may result in disciplinary action including, but not limited to, being sent home at parent/guardian expense (parent/guardian to be responsible for all financial consequences of student behavior), as well as possible further disciplinary action deemed appropriate by the school after the conclusion of the trip.

I, THE UNDERSIGNED, HEREBY DECLARE THAT I have read, and understand the above contract for off-campus travel. I will comply with these expectations, including any rules and regulations. I will respect group leaders’ instructions and wishes, understanding that these are for my safety and to protect me. I know, understand and declare that I will abide by this value contract at all times. The consequences for not following this contract may result in being sent home and expulsion from school. In case of harm and/or injury occurred and/or sustained by not following a group leader’s instruction and guidance, I and my family will cover all the necessary expenses/cost. This statement applies to property damage, as well as any expenses related to physical harm and/or injury to myself and others.

 

Ⅲ. Permission to seek treatment and dispense medication

※ This form must be submitted as a condition of enrollment.

I do hereby give my consent for my child to receive treatment for illness or accidents that may occur during their time at KISJ. The faculty of KISJ who are currently certified in First Aid/CPR/AED and/or Lifeguarding has my permission to help my child in the event of an emergency at KISJ, before more highly qualified professionals arrive, such as EMS. The faculty at KISJ has my permission to take my child to the doctor or hospital, purchase the prescribed medication, and dispense medications as recommended by the doctor. I understand that KISJ will not be held responsible for any accidents that may occur due to the care provided by a trained professional and/or the use of this medication that has been prescribed by a certified doctor. I also understand that I will be billed for any costs incurred for medical services.

 

Ⅳ. Medical Authorization Form

※ This form must be submitted if and when your child needs medication to be administered at KISJ.


I do hereby give my consent for my child to receive the following medication at school, administered by the school nurse. I understand that KISJ will not be held responsible for any accidents that may occur due to the use of medication that I have provided for my child.

If the student doesn't need the medication, you can leave it blank.

I understand that this medicine must be sent to school in a fully labeled container, preferably the original labeled container from the store or pharmacy.

 

V. Permission about the KISJ Respiratory Illness & COVID-19 Response Guidelines

※ This form must be submitted as a condition of enrollment.

I do hereby give my consent for my child within the school protocol guideline about respiratory illness & COVID-19 response. I agree with submitting all the documents needed when the school nurse team requests. I also understand that I will be billed for any costs incurred for medical services when needed.

VI. Acknowledgement and Parent/Legal Guardian Agreement for Korea International School, Jeju Campus (KISJ) School and Dorm Life Guide 2024-25

  1. The parent/guardian acknowledges that the information contained in the KISJ School and Dorm Life Guide 2024–25 has been read and is accepted.
  2. The parent/guardian agrees with the payment of all fees as established by KISJ, and in accordance with authorized payment plans must be paid in full by the date established. In the case of non-payment, KISJ may restrict students from attending classes, withhold report cards, and/or refuse re-enrollment.
  3. The parent/guardian agrees to ensure that their student attends all regularly scheduled classes. In the event of an absence, the parent/guardian understands that supporting documents of explanation for the absence must be submitted.
  4. The parent/guardian agrees to support their student in order to help them meet KISJ’s academic and behavioral standards.
  5. The parent/guardian agrees to cooperate and follow the directions of teachers and administrators in order to support the progress of their student.
  6. The parent/guardian agrees to use proper channels of communication established by KISJ when seeking resolution to a problem or concern (i.e. initial contact with the teacher or person closest to the problem and then, if necessary, forwarding the matter to the principal or head of boarding).
  7. The parent/guardian understands that transportation services, bakery and cafeteria, and laundry services are provided by a third party contractor. The parent/guardian agrees to comply with the policies and financial obligations set forth by the third parties. KISJ is considered only an intermediary between the two parties.
  8. The parent/guardian understands their responsibility to contribute to a positive school environment and avoid conduct that creates or promotes intolerance or disharmony between and/or among the members of the school community and agrees to avoid making unfounded comments that may damage the image of the school or staff.
  9. Failure to comply with this agreement and the policies and guidelines of Korea International School, Jeju Campus may result in a student’s withdrawal from the school or dorm.

 

Our names and signatures indicate we have received, read, understand and agree to abide by all policies set forth in the Korea International School, Jeju Campus School and Dorm Life Guide 2024–25.

  • I will communicate positively when completing assignments using digital apps for academic purposes.

  • I will use technology in a respectful way, with  purpose, and  at the appropriate times.
  • I understand that the school network is monitored and controlled by the IT Department, and will not undermine or avoid this through any means. 
  • I understand that use of technology at school is not a right but a privilege and will always take great care of all technology equipment - the school’s, my peers’ and my own.

 

Responsibility

  • I will manage my digital identity and reputation. I am aware of the permanent impact of my actions in the digital world.
  • I will only engage in positive, safe, legal and ethical behavior when using technology, including social interactions online or when using networked devices.
  • I will manage all personal data to maintain digital privacy and security (for myself and others).
  • I understand and respect the rights and obligations of using and sharing intellectual property.

 

Collaboration
