KISJ Waiver Agreement

This form must be signed by the parent and returned to the KISJ administrative office.

1. I, the undersigned, understand that I am responsible for my child’s health and safety.

Having such knowledge, I hereby agree to release KISJ from all liability for any accidental

injury and/or harm which may occur/sustained as a result of my child’s action.

2. I hereby consent to indemnify the necessary expenses/cost.

This applies to property damage, as well as any expenses related to physical

harm and/or injury.

3. In signing this release, I acknowledge and represent that I have read the KISJ

Waiver Agreement form, understand it and sign it voluntarily.
